​New Patient Information



To make an appointment call (336) 765-9314 between 8:15am and 4:30pm Monday through Thursday, 8:15am and 4:00pm Friday. If you were seen in the Emergency Department at Forsyth Medical Center, please call as soon as possible.and were instructed to make an appointment with one of our doctors, please call as soon as possible.



New patients will find forms to be filled out and bring with you. Please be sure when coming for an appointment to bring any x-rays, MRIs, CT scans, bone scans or any other studies you have had pertaining to your complaint. Please fax or bring any physician notes or reports addressing the problems at hand. You will also be required to bring your current insurance cards as well as a list of current medications.



If you had any x-rays or other studies conducted at Forsyth Medical Center’s Emergency Department, please be sure to bring a CD of those images. If you did not receive this CD upon your departure, please call (336) 718-5869 to have the CDs made and ready for pick up. Please be advised that the hospital may tell you that we can access those images through our computers, but computers are not fail-proof and we do not want to delay your being seen because we cannot access your studies.

First time patients seeking an appointment, please keep these things in mind:

• A referral from a family physician is NOT required.

• We do accept Tricare insurance. Such patients, however, must have a Tricare referral (if required) from his or her family doctor.

• Cigna HealthSprings will require referral from primary care physician.

• We do not accept local second opinions, however if you are coming from another city or state we will accept notes and reports for review before making a decision.

• If calling for an appointment related to back pain, please be prepared to send notes from a family doctor for review.

• Winston Bone and Joint accepts the majority of insurance carries, excluding Medicaid, Starbridge Beach Street and out of state Coventry.
